Paroxysmal supraventricular tachycardia (PSVT) is episodes of rapid heart rate that start in a part of the heart above the ventricles. “Paroxysmal” means from time to time.

  • Major comorbidities:

    IDENTIFICATION AT TRIAGE- ECG, THEN INTO RESUS. MONITORING

  • STABLE PATIENT.
  • PATIENT COMPLIANT WITH LYING FLAT? YES/NO
  • VALSALVA IN RECUMBENT POSTURE. SUPINE WITH LEGS ELEVATED
    TECHNIQUE: MAXIMAL STRAIN BY BLOWING INTO 10 ML SYRINGE TO PUSH PLUNGER OUT FOR 15 SEC, WAIT ONE MINUTE AFTER,

SUCCESS: YES/NO (REVERSION TO SINUS RHYTHM)

  • REPEAT ABOVE AGAIN. SUCCESS: YES/NO
  • IF NO REVERSION TO SINUS RHYTHM
    ADENSINE 6,6, 12 MG IN SUCCESSION   SUCCESS: YES/NO
  • IF NO REVERSION TO SINUS RHYTHM – CARDIOLOGY CONSULT
